Medical Reports

Commercial Drivers

Commercial drivers who were required to submit a medical report on or after March 1, 2020, as part of maintaining their Commercial driver’s licence, were required to submit their medical report by October 31, 2021.

If your report was not received by October 31, 2021, your Commercial driver’s licence may have been downgraded.

If you have questions about the status of your medical report or driver’s licence, please contact Driver Medical Review Office at 416-235-3400  or toll-free at Toll-free: 1-800-304-7889.

Drivers with a Reported Medical Condition

Drivers (all licence classes) who have been asked to submit a medical report to keep their current driver’s licence need to submit a completed, current medical report. Your driver’s licence may be downgraded or suspended if you do not submit the medical report.

If you need to submit a medical report to avoid having your licence suspended or downgraded, you will have received a letter and a medical report to be completed by your healthcare practitioner. Have the report completed by your doctor, specialist or nurse practitioner and sent to the Driver Medical Review Office.
