Overview

Health care professionals must complete the Medical Review Package to help us determine whether a person continues to meet the definition of a person with a disability for the purposes of the Ontario Disability Support Program (ODSP). The Ministry of Children, Community and Social Services uses this set of forms to determine if your patient still qualifies for ODSP income support.

If they have a medical review date assigned, patients might bring you the Medical Review Package to complete.

To avoid delays for your patient, it is important to:

  • complete all sections and questions as fully as possible
  • include duration for all condition(s) and resulting impairment(s)
  • sign all appropriate pages
  • attach additional medical and supporting information, such as clinical notes, hospital reports or consult reports, that support or clarify the information you identify in the forms

As of August 2021, there is an updated Self Report form to submit with the Medical Review Package should patients wish to complete one. The medical review process and definition of disability have not changed.

Your role as a health care professional

As a licensed health care professional in Ontario, you may be asked to provide medical information about their conditions, impairments and restrictions to help us decide if your patient still qualifies for ODSP income support. Your role in completing the Medical Review Package is important since we do not assess your patient in person.

A medical review is not a new application. It focuses on changes to your patient’s original medical conditions or any new medical issues.

Specialized staff at the Disability Adjudication Unit will determine if your patient continues to be medically eligible for ODSP based on the information you provide.

After you complete the Medical Review Package, please return the completed forms to your patient.

How patients can submit their forms

Your patient is responsible for mailing the completed original forms to the Disability Adjudication Unit using the addressed envelope included with their package. We will only accept original forms.

Timelines

Your patient has 90 calendar days to submit their completed MRP and submit it to us. If they need more time to complete the package, they can contact the Disability Adjudication Unit to ask for an extension.

About the forms

Part A

Part A is mandatory and collects current information about the medical conditions that were originally identified in the previous disability determination package.

Part B

Part B identifies medical conditions that are not listed in Part A. It has two sections:

  • the Health Status Report (HSR)
  • Activities of Daily Living (ADL)

This section is only completed if necessary, based on the answers to the questions asked at the end of Part A.

Summary of disability decision

You will also receive a summary of the disability decision. This summary provides the reasons your patient was originally found to be a person with a disability and may assist you in completing the forms.

Who can complete the forms

Part A and Part B (both Health Status Report and the Activities of Daily Living) can be completed by a:

  • physician
  • optometrist
  • psychologist
  • psychological associate
  • nurse practitioner
  • registered nurse

The Activities of Daily Living in Part B can also be completed by a:

  • chiropractor
  • physiotherapist
  • occupational therapist
  • social worker
  • audiologist
  • speech language pathologist

Self Report form

Your patient may also complete the Self Report form which allows them to explain how their disability impacts their life. Please encourage them to fill it out so the ministry can better understand their situation. Your patient may ask someone for help completing the form if needed.

How to bill for completing the forms

Physicians can bill the Ontario Health Insurance Plan (OHIP) directly. Other health care professionals can send us an invoice.

Submitting an invoice

If you submit an invoice, include:

  • your full name, profession, address and phone number
  • your patient's full name, date of birth and social assistance member ID (this is on each page of the form)
  • the name of the section you completed (HSR and/or ADL)
