Take notice that any person having claim arising out of a Load Brokerage Service against the above Load Brokers, who carried on business in the Province of Ontario, is required to file a claim together with a copy of a court judgement with the Deputy Registrar of Motor Vehicles at:
301 St. Paul Street, 3rd Floor
St. Catharines, ON L2R 7R4
On or before April 15, 2004.
Note: Where the dollar value of the payable claims exceed the dollar value of the forfeited bond ($10,000.00 each) the payment will be made on a pro rata basis.
