The Association of Ontario Land Surveyors have approved a new Plan Submission Form. This new form, which is on a clear material, will replace the "blue sticker" Plan Submission Form, currently in use. A copy of this new form is shown below.

The intention of the form remains the same for the purposes of the Land Registration System. All plans of survey tendered for deposit/registration must be accompanied by a print of the plan bearing a Plan Submission Form and these prints are still to be forwarded to the AOLS.

The Association of Ontario Land Surveyors has advised that they will discontinue ordering the "blue" form. However, in order to accommodate those surveyors that have purchased a supply of the old "blue" forms, either Plan Submission Form will be accepted.
