ONTARIO REGULATION 511/01

FEES FOR SERVICES PROVIDED BY THE REGISTRAR GENERAL

Registration more than one year after birth

1. The fee for registration of the birth of a child that has not been registered within one year from the date of birth is $50. O. Reg. 511/01, s. 1.

Change of name certificate

2. The fee for a change of name certificate is $15. O. Reg. 511/01, s. 2.

Birth certificate

3. (1) The fee for a birth certificate is $25. O. Reg. 511/01, s. 3 (1).

(2) The fee for a replacement birth certificate is $35. O. Reg. 511/01, s. 3 (2).

Marriage certificate

4. The fee for a marriage certificate is $15. O. Reg. 511/01, s. 4.

Death certificate

5. The fee for a death certificate is $15. O. Reg. 511/01, s. 5.

Certified copy of birth registration

6. (1) The fee for a certified copy of a registration of birth is $35. O. Reg. 511/01, s. 6 (1).

(2) The fee for a replacement certified copy of a registration of birth is $45. O. Reg. 511/01, s. 6 (2).

Other certified copies

7. The fee for a certified copy of a registration of marriage, death or still-birth is $22. O. Reg. 511/01, s. 7.

Fee surcharge

8. The fee payable for a service provided by the Registrar General under the Act is increased by $15 if,

(a) the person who requests the service requests that it be provided by the end of the second business day following the day the request is made; and

(b) the service is provided to the person by the end of the second business day following the day the request is made. O. Reg. 430/03, s. 1.

Waiver of payment of fee

9. (1) The payment of the fee payable under section 3 for a birth certificate or replacement birth certificate is waived for a homeless or marginally-housed individual in Ontario whose application for a birth certificate or replacement birth certificate is forwarded to the office of the Registrar General by a corporation that meets the requirements set out in subsection (2). O. Reg. 163/05, s. 1.

(2) The corporation referred to in subsection (1) must meet the following requirements:

1. The corporation must be a non-profit corporation.

2. The corporation must have a mandate to assist homeless or marginally-housed individuals in Ontario to obtain a birth certificate or replacement birth certificate, in order to assist those individuals to access necessary services such as health care, shelter, housing, food, mental health services and social assistance.

3. There must be no other public funding available to the corporation for the payment of the fee for the birth certificate or replacement birth certificate.

4. The corporation must be a party to an agreement with the Deputy Registrar General for a definite term, under which the corporation agrees that,

i. the corporation will not seek the waiver of the payment of the fee for a birth certificate or replacement birth certificate for an individual, unless it believes that the individual is homeless or marginally-housed and that the individual needs the birth certificate or replacement birth certificate in order to assist the individual to access necessary services such as health care, shelter, housing, food, mental health services and social assistance,

ii. the corporation will not disclose to anyone other than the office of the Registrar General the identity of, or any other personal information about, an individual whose application for a birth certificate or replacement birth certificate the corporation forwards to the office of the Registrar General, unless the corporation first obtains the written consent of the individual or a person authorized to consent on behalf of the individual,

iii. the corporation will permit the Deputy Registrar General or a person designated by the Deputy Registrar General to enter the offices of the corporation for the purpose of making inspections and inquiries in order to determine whether the corporation is complying with the agreement, and

iv. the Deputy Registrar General may terminate the agreement if the Deputy Registrar General believes that the corporation does not meet a requirement in paragraph 1, 2 or 3 or has not complied with subparagraph i, ii or iii. O. Reg. 163/05, s. 1.

(3) Subsection (1) does not waive the payment of any surcharge payable under section 8. O. Reg. 163/05, s. 1.
