ONTARIO REGULATION 80/19

MATTERS RELATED TO FUNCTIONS UNDER SUBSECTION 14 (1.1)

Provision of information

1. (1) A children’s aid society or a residential licensee shall inform the Ombudsman in writing and without unreasonable delay if the society or licensee learns of the death of or serious bodily harm to a child or a young person who had sought or received a children’s aid society service within 12 months before the death or the day on which the harm occurred.

(2) In informing the Ombudsman the children’s aid society or residential licensee shall provide a summary of the circumstances surrounding the death or serious bodily harm.

(3) A children’s aid society or a residential licensee that is required to inform the Ombudsman about the death of or serious bodily harm to a child under subsection (1) shall also inform any person with lawful custody of or a lawful right of access to the child about the Ombudsman and provide to the person the Ombudsman’s contact information.

(4) A children’s aid society or a residential licensee that is required to inform the Ombudsman about serious bodily harm to a child or a young person under subsection (1) shall also inform the child or young person about the Ombudsman and provide to the child or young person the Ombudsman’s contact information.
