Amending Reg. 569 of R.R.O. 1990
(REPORTS)
1. Subsection 1 (3) of Regulation 569 of the Revised Regulations of Ontario, 1990 is amended by striking out “Form 3” at the end and substituting “the form entitled “Form 3 – Notification of New Active – Leprosy (Hansen’s Disease)”, dated January 2017 and available on the website of the Government of Ontario Central Forms Repository under the listing for the Ministry of Health and Long-Term Care”.
2. The French version of subparagraph 1 iii of section 5 of the Regulation is amended by striking out “si la personne est hospitalisée” and substituting “si la personne est admise dans un hôpital”.
3. Form 3 of the Regulation is revoked.
