Overview

The Ontario Laboratories Information System (OLIS) is an electronic system that gives authorized health care providers access to patient lab test orders and results from hospitals and community labs.

As patients move between health care providers such as hospitals, primary care providers, home care and long-term care settings, OLIS makes viewing patients’ current and past test results faster and easier and enables treatment decisions to be made at the point-of-care.

How it works

  1. Laboratories that perform lab testing submit your test results back to the health care provider who ordered the test(s) and any other health care provider copied on the lab requisition form.
  2. Laboratories also submit your test results to OLIS.
  3. The Ministry of Health (the ministry) has custody and control of the test results in OLIS and provides those test results to Ontario Health, the public agency that provides a secure and private electronic health care system for all Ontarians.
  4. Ontario Health can then share your information electronically with any of your health care providers within Ontario.

Protecting your privacy

The ministry, Ontario Health and the collection, use and disclosure of patient personal health information are all subject to the Personal Health Information Protection Act (PHIPA).

The ministry and Ontario Health will only disclose your information as it is required by your health care provider, and as permitted by law to ensure that your information is secure, and your privacy is respected.

Restrict or reinstate access

As a patient, you can withdraw or reinstate your consent for the ministry and/or health care providers who have not ordered your laboratory tests to access your laboratory test information contained in OLIS.

If you choose to withdraw your consent, the ministry and/or health care providers who have not ordered your laboratory tests will not have access to this information through OLIS.

Choosing to withdraw your consent will in no way affect your relationship with your health care provider(s), who will continue to provide care to you.

Restrict access to all your information

You can restrict access to all your information by:

  1. Completing the Withdrawal of Consent Form
  2. Submitting the completed form in person to a laboratory participating in OLIS when having a test performed.

Alternatively, you can contact ServiceOntario to restrict access to your information by calling at 1-800-291-1405 (TTY: 1-800-387-5559).

Restrict access to a specific laboratory order

You can restrict access to a specific laboratory order by:

  1. Checking with your laboratory to see if they can restrict access to a specific laboratory order.
  2. Clearly identifying your preference in the test information section of the Withdrawal of Consent Form.
  3. Submitting the completed form in person to the laboratory.

Reinstate access to all your information

You can reinstate access to all your information by calling ServiceOntario at 1-800-291-1405 (TTY: 1-800-387-5559) at your convenience.

Your consent cannot be retroactively reinstated for specific laboratories or laboratory orders.
