Overview

You can register a vital event, such as a birth, death, marriage or stillbirth, that occurred in Ontario, apply for a name change, apply to amend or correct information on a vital event registration and get your official government certificates from the Office of the Registrar General through ServiceOntario.

When requesting vital event services from the Office of the Registrar General, you may be asked to submit additional information using one of the forms on this page.

Certificate applications and urgent requests

Submit additional documents and information and make a payment for:

  • existing applications for birth, marriage or death certificates
  • urgent certificate requests where additional documentation is required (for example, court orders)
  • urgent requests for certified copies of death registration with cause of death information
  • birth certificates displaying “X” in the sex designation field
  • birth certificates that do not display the sex designation field

Note: Do not include credit card information in your application form. Once you have completed your form, you will be directed to a payment page where you can submit your payment information and complete your request.

Contact the Office of the Registrar General to request an application form be mailed to you to:

  • amend the registration of a:
    • birth
    • death
    • marriage
    • stillbirth
  • change a name

Birth, marriage and death registrations

Hospitals, eligible municipalities and funeral homes can request registration services to:

  • update or submit additional information for an existing birth, death, marriage or stillbirth registration or delayed registration
  • submit additional information for medical coding purposes

Locating death records and Indian Residential Schools

Indigenous communities, organizations or family members trying to locate death records, or registrations of deaths, for children who attended Indian Residential Schools can request a search for a death record for no fee.
