Accelerating Ontario Health Teams to deliver better care

The Ontario government is supporting Ontario Health Teams (OHTs) as they continue to better connect care and improve patient experience in their local communities.

To this end, the government is investing an additional $9.2 million in fiscal year 2023-24 to accelerate OHT progress.

Up to $6.2 million of this funding will support 12 teams selected to rapidly implement improvements to patient care, and governance and operational structures. This includes:

  • Implementation of primary care-based chronic disease care models that support people with multiple or complex chronic conditions
  • Establishment of primary care networks, in accordance with ministry guidance
  • Implementation and uptake of local digital tools that support virtual care.

As the initial 12 OHTs identified for acceleration take on additional responsibilities, they will test new models of care, share lessons learned, and provide peer leadership to support accelerated impact across all OHTs.

Up to $3 million of this funding will support all OHTs in Northern Ontario to meet their communities’ needs. This funding will support Northern OHTs as they advance a more connected health care system in the face of unique circumstances such as remote locations and provider dispersion.    

These additional investments build on the government’s previously announced commitment to provide longer-term funding for all OHTs of up to $2.2 million per OHT over three years beginning in fiscal year 2024-25 so that they can continue to mature and work towards potential designation under the Connecting Care Act, 2019.

The Ministry of Health and Ontario Health will continue to collaboratively work with OHTs to inform guidance on expectations laid out in The Path Forward.
