Amending O. Reg. 168/15

(GENERAL)

1. (1) Table 2 of Ontario Regulation 168/15 is amended by adding the following rows:

 

Alpha Lipoic Acid

Intravenous

Maximum daily dose of 600 mg for racemic form or 300 mg for R form.

Intravenous

Must be in combination with other amino acids.

 

(2) Table 2 of the Regulation is amended by striking out the rows beginning with “Vitamin B1” through “Vitamin B6” in the first column and substituting the following:

2. Table 3 of the Regulation is amended by striking out the row that begins with “Vitamin D” in the first column and substituting the following:

 

Vitamin D

Only if prescribed in oral dosage form containing more than 2,500 International Units of Vitamin D per dosage form or where the largest recommended daily dosage shown on the label would result in the daily intake by that patient of more than 2,500 International Units of Vitamin D.

 

3. Table 4 of the Regulation is amended by striking out the row that begins with “Vitamin D” in the first column and substituting the following:

 

Vitamin D

Only if dispensed in oral dosage form containing more than 2,500 International Units of Vitamin D per dosage form or where the largest recommended daily dosage shown on the label would result in the daily intake by that patient of more than 2,500 International Units of Vitamin D.

 

4. Table 5 of the Regulation is amended by striking out the row that begins with “Vitamin D” in the first column and substituting the following:

Only if compounded in oral dosage form containing more than 2,500 International Units of Vitamin D per dosage form or where the largest recommended daily dosage shown on the label would result in the daily intake by that patient of more than 2,500 International Units of Vitamin D.

 

5. Table 6 of the Regulation is amended by striking out the row that begins with “Vitamin D” in the first column and substituting the following:

Only if sold in oral dosage form containing more than 2,500 International Units of Vitamin D per dosage form or where the largest recommended daily dosage shown on the label would result in the daily intake by that patient of more than 2,500 International Units of Vitamin D.

6. This Regulation comes into force on the day it is filed.
