From January-September of 2019, an audit of the Office of the Chief Coroner and the Ontario Forensic Pathology Service (OCC/OFPS) was conducted by the Auditor General’s (AG) office in keeping with its mandate to evaluate the effectiveness of the Ontario Public Service. The completed audit report was released in December 2019. The report provided recommendations to the OCC/OFPS to enhance their service.

In January 2020, a project management team was assembled to implement the pathology-related recommendations of the AG’s report as well from a few other external reviews. In total, there are 35 recommendations to adopt into OFPS practice that address the following themes:

  • Quality Assurance
  • Pathologist Register
  • Scene Attendance
  • Body Management
  • Transfer Payment and Invoicing
  • Policy and Training
